Knowledge Stream vs Competency Stream: Choosing Your Australian Pharmacy Registration Pathway

Australia has two distinct pathways for international pharmacists seeking registration — the Knowledge Stream and the Competency Stream. One of the most common questions international pharmacists ask is "which pathway should I choose?" The reality is that you usually do not get to choose. Your country of qualification and registration status determine which pathway you are eligible for.

This comprehensive guide explains both pathways in detail, compares them side by side, and helps you understand exactly what each involves from start to finish.

The Two Pathways: An Overview

The Australian Pharmacy Council (APC) administers the skills assessment process for international pharmacists. There are two routes:

Knowledge Stream

  • Exam: OPRA (Overseas Pharmacist Readiness Assessment)
  • For: Pharmacists who qualified in most countries worldwide
  • Focus: Tests pharmaceutical knowledge and its application to Australian practice

Competency Stream

  • Exam: CAOP (Competency Assessment for Overseas Pharmacists)
  • For: Pharmacists from specific countries with similar regulatory frameworks to Australia
  • Focus: Tests whether your existing competency transfers to the Australian context

Both pathways ultimately lead to the same destination: a positive Skills Assessment Outcome, which allows you to apply for provisional registration with AHPRA and begin an internship in Australia.

Knowledge Stream: The Full Pathway

Who Is Eligible?

The Knowledge Stream applies to pharmacy graduates from all countries except:

  • Australia
  • Canada
  • Ireland
  • New Zealand
  • United Kingdom
  • United States

This includes pharmacists from India, Philippines, Egypt, South Africa, Pakistan, Bangladesh, Sri Lanka, Nepal, Middle Eastern countries, and all other nations not listed above.

Step-by-Step Process

Stage 1: Eligibility Check — $810 AUD

You submit your qualifications and documentation to the APC for assessment.

Requirements:

  • Pharmacy qualification of at least 4 years full-time (post-2006) or 3 years (pre-2006)
  • Evidence of registration or eligibility to register in your home country
  • Official academic transcripts
  • Degree certificate
  • Photo identification (passport)
  • NAATI-certified English translations of all non-English documents

Processing time: Target of 5 working days, but can take up to 4 weeks during busy periods.

Outcome: You receive confirmation of eligibility to sit the OPRA exam, or a request for additional documentation.

Stage 2: OPRA Exam — $2,245 AUD

The main assessment of the Knowledge Stream.

FeatureDetail
Questions120 MCQs
Duration2.5 hours (150 minutes)
DeliveryComputer-based at Pearson VUE test centres
FormatClosed-book (no references allowed)
Availability80+ test centres across 28 countries
Scoringscaled scoring system
Content focusTherapeutics (~45%), pharmaceutical sciences (~20%), pharmacy practice (~15%), calculations (~10%), professional practice (~10%)

Key point: OPRA is closed-book. You cannot bring any reference materials into the exam. This is different from the Intern Written Exam (which is open-book from 2026).

Stage 3: Skills Assessment Outcome — $300 AUD

After passing OPRA, the APC issues your Skills Assessment Outcome document. This confirms you have met the skills assessment requirements and is valid for 3 years for visa and registration purposes.

Total APC Knowledge Stream fees: $3,355 AUD

Stage 4: AHPRA Provisional Registration — $484 AUD ($583 for NSW)

With your positive Skills Assessment Outcome, you apply to the Australian Health Practitioner Regulation Agency (AHPRA) for provisional registration. This allows you to practise as a pharmacy intern under supervision.

Stage 5: Intern Year

You must complete:

  • An accredited intern training programme ($3,000–$8,000 depending on provider)
  • A minimum of 1,824 hours of supervised practice (equivalent to approximately 12 months full-time)
  • Documentation of competency development across six domains

Stage 6: Intern Written Exam — ~$790 AUD

A competency-based exam that all interns (including Australian graduates) must pass:

  • 75 questions, 2 hours
  • Open-book (AMH + APF allowed from 2026)
  • 65% scaled pass mark
  • MCQ + fill-in-the-blank calculations

Stage 7: Intern Oral Exam — ~$700 AUD

The oral exam has two components:

  • Practice Component (~$475): Clinical practice scenarios assessed through structured oral examination
  • Law & Ethics Component (~$225): Legal and ethical reasoning in pharmacy practice scenarios

Stage 8: General Registration — $484 AUD ($583 for NSW)

After passing all exams and completing your supervised practice, you apply for general registration with AHPRA. You are now a fully registered pharmacist in Australia.

Competency Stream: The Full Pathway

Who Is Eligible?

The Competency Stream applies to pharmacists who meet both of these criteria:

  1. Completed their pharmacy qualification in Canada, Ireland, United Kingdom, or United States
  2. Hold current registration in one of these countries at the time of application

Important: Past registration does not count. You must be currently registered when you apply. If your registration in your qualifying country has lapsed, you need to reinstate it before applying for the Competency Stream.

Why These Countries?

These four countries have pharmacy regulatory and education frameworks that are considered substantially similar to Australia's. The pharmacy curricula, practice standards, and regulatory structures in Canada, Ireland, the UK, and the USA share enough common ground with Australia that a full pharmaceutical knowledge assessment (like OPRA) is not considered necessary.

Instead, the CAOP assesses whether your existing competency — demonstrated by your current registration — transfers effectively to the Australian practice context.

Step-by-Step Process

Stage 1: Eligibility Check — $810 AUD

Similar documentation requirements to the Knowledge Stream:

  • Proof of pharmacy qualification from a qualifying country
  • Evidence of current registration in a qualifying country
  • Photo identification
  • Academic transcripts (usually already in English)

Processing time: Target of 5 working days.

Stage 2: CAOP Exam — $2,100 AUD

The Competency Assessment for Overseas Pharmacists.

Key advantage: The CAOP is available online via OnVUE — you can sit it from home with remote proctoring. This is a significant convenience factor, especially for candidates who are not yet in Australia.

Stage 3: Skills Assessment Outcome — $300 AUD

Same as the Knowledge Stream — issued after passing CAOP.

Total APC Competency Stream fees: $3,210 AUD

Stages 4–8: Same as Knowledge Stream

After the Skills Assessment Outcome, the pathway is identical:

  • AHPRA provisional registration
  • Intern year (training programme + supervised practice)
  • Intern Written Exam
  • Intern Oral Exam
  • General registration

  • Pass/Fail only — no performance feedback
  • If your registration lapses before applying, you must reinstate it first

Which Pathway Should You Choose?

In most cases, you do not choose — your country of qualification determines your pathway. However, there are some edge cases:

If You Qualified in a Non-Qualifying Country but Are Registered in a Qualifying Country

For example, if you completed your pharmacy degree in India but are currently registered in the UK. In this case, your country of qualification matters — check with the APC directly, as the eligibility criteria are based on where you obtained your pharmacy degree, not where you are currently registered.

If You Qualified in a Qualifying Country but Your Registration Has Lapsed

You would need to reinstate your registration before applying for the Competency Stream. If reinstatement is not possible or practical, contact the APC to discuss your options.

If You Are a New Zealand Graduate

New Zealand pharmacists fall under special arrangements due to the Trans-Tasman Mutual Recognition Act. Contact the APC or Pharmacy Board directly — your pathway may be different from both streams described above.

Frequently Asked Questions

Can I choose between the Knowledge Stream and Competency Stream?

In most cases, no. Your pathway is determined by where you obtained your pharmacy qualification. If you graduated from a pharmacy programme in Canada, Ireland, the UK, or the USA and hold current registration there, you are eligible for the Competency Stream. All other international pharmacists are assessed through the Knowledge Stream.

What is the difference between OPRA and CAOP?

OPRA (Overseas Pharmacist Readiness Assessment) is the Knowledge Stream exam — 120 MCQs, 2.5 hours, closed-book, $2,245, testing pharmaceutical knowledge and therapeutics. CAOP (Competency Assessment for Overseas Pharmacists) is the Competency Stream exam — 70 questions, 2 hours, $2,100, testing competency transfer, with an online exam option available.

Do both pathways require an internship?

Yes. Both the Knowledge Stream and Competency Stream lead to a Skills Assessment Outcome, after which you must complete an intern year. This includes an accredited intern training programme, a minimum of 1,824 hours of supervised practice, the Intern Written Exam, and the Intern Oral Exam before you can apply for general registration.

Is the Competency Stream easier than the Knowledge Stream?

Neither pathway is inherently easier — they assess different things. The Competency Stream assumes you trained in a system similar to Australia's and tests whether that competency transfers. The Knowledge Stream comprehensively tests pharmaceutical knowledge and its application. Each is calibrated for its target candidate population.

Which pathway is cheaper?

The Competency Stream is typically less expensive overall. The CAOP exam is $140 cheaper than OPRA, and Competency Stream candidates from English-speaking countries often do not need English language testing ($400–$1,800 saved) or document translation ($200–$700 saved). The difference can be $740 to $2,640 in total savings.

What happens after I pass the skills assessment exam?

After passing either OPRA or CAOP, you receive a Skills Assessment Outcome from the APC. You then apply to AHPRA for provisional registration, which allows you to begin your intern year in Australia. During the internship, you complete supervised practice and an accredited training programme, then sit the Intern Written Exam and Intern Oral Exam. Upon passing, you apply for general registration.

Can I sit CAOP online?

Yes. The CAOP exam is available online via OnVUE proctoring, which means you can sit it from home or any suitable location with a computer and internet connection. The OPRA exam does not have an online option — it must be sat in person at a Pearson VUE test centre.

What if I qualified in New Zealand?

New Zealand pharmacists are subject to special arrangements under the Trans-Tasman Mutual Recognition Act. The standard Knowledge Stream and Competency Stream pathways may not apply. Contact the Australian Pharmacy Council or the Pharmacy Board of Australia directly to understand your specific registration pathway.
